Det Sgt Teunis asked Det Howard and me to assist New Port Richey PD in finding background and next-of-kin on victim Lisa McPherson as she had died in a Hospital in New Port Richey but reportedly lived in Clearwater. A possible address of 901 No Osceola #205 was given. We checked that location but found that it is a bad address --- would fall approximately where the back of North Ward Elementary School sits.

We were advised also that one of the persons who transported her to the pital may be a Janis Johnson who gave a phone number of 461-4975. (This r-one number is listed to a Kevin L Wiley at 551 No Saturn Ave, Clearwater) I telephoned Ms Johnson to make sure she was home.

Arrangements were made to meet with her by the complex pool. We met her at approx 1235 hours. She said she is an M.D. from another state but does not practice medicine here in Florida. She is called on when someone in the Church (of Scientology) gets sick to give her opinion as to whether or not the person should go to a Doctor or a Hospital. On this occasion she was called by Alain Kartuzinski who is a Supervisor at 210 So Ft Harrison, "The Ft Harrison". When she arrived, McPherson was in a room and was awake. She said "Hill to Johnson then indicated that her throat was hurting. Johnson said right away she could see that McPherson was very sick, saying she looked SEPTIC. She called Dr Minkoff as he is the Doctor used by the Church and decided she should take McPherson to him. She asked Laura Arrunada to go with her as Laura has a Medical degree from Mexico. She then asked if anyone else around had medical training to help her transport and Paul Greenwood said he had Chiropractic training, so Johnson asked him to help lifting McPherson in and out of her They took McPherson to HCA Hospital in New Port Richey as that is where Minkoff works. Det Howard asked why'they didn't go to Morton Plant Hospital as it was much closer if she was so sick. Johnson's reply was "I didn't think she was that sick".

She has heard that McPherson is divorced and that her Mother lives in or near Dallas. However she was able to furnish no further personal information on her.

Det Howard and I next went to AMC Publishing at 620 Lakeview Road, Clearwater, where McPherson had reportedly been working. We were told that Bennetta Slaughter, the supervisor, was out of the office, but we were able to speak with her husband David Slaughter. He said Bennetta is the one we would need to talk with as she has the personnel files but he would attempt to give us some background information. He said McPherson's Mother does in fact live in Dallas and that her first name is Fannie but said he doesn't know her last name and thinks it is different from McPherson's. He said McPherson has worked for this Company for several years and is a personal friend of his and his wife's but said he didn't know much about her. He did she was in sales in this off ice and had been out sick that past couple of ks with unknown ailment. He took my business card and agreed to call or have his wife call.

When I arrived for work on Thursday morning, 7 December, there was a message on my recorder from David Slaughter. He said he had found out that McPherson's home address was 240 Windward Passage #302. He said he still hadn't found out how to reach her Mother but said he had other information on her if I would call him. He left a home number of 581-9444 and said his office number is 446-1100 --- works 9AM - 6PM. I attempted to call him at work but was told he was in a meeting. The receptionist said she would ask him to call me back.

This concluded my involvement to date.

Victim Lisa McPherson was a 36 year old white female born on February 10, 1959, in Dallas, Texas. At the time of her death, Lisa was survived by her mother, Fannie McPherson. Lisa's brother committed suicide when Lisa was approximately 16 years of age. Lisa's father also committed suicide some years after her brothers death. At the age of 18, and while residing in Dallas, Texas, Lisa McPherson was introduced into the Church of Scientology by a co-worker while she was employed at the local phone company. At the time of her death, Lisa was employed at AMC Publishing Company as a sales representative selling insurance advertising packages. Lisa had been employed at AMC Publishing since the late 1980's and relocated to Clearwater, Florida in 1993 with other AMC employees.

AMC Publishing Company is owned and operated by fellow Scientologists.

During her last 18 years Lisa was a dedicated member of the Church of Scientology and had acted in both "staff" and "public" positions within the Church of Scientology. It has been reported that Lisa's social, professional and personal life was solely dedicated.to the Church of Scientology.

Friends described Lisa McPherson as a likable, extroverted individual who had the gift for gab and thus, was the highest producing sales representative at AMC Publishing.

On Saturday, November 18, 1995, Victim Lisa McPherson was involved in a minor automobile accident near the intersection of South Fort Harrison and Belleview Boulevard in Clearwater, Florida. McPherson rear-ended a vehicle towing a boat which had stopped in the curb lane. Several other individuals were at the crash scene as another accident had previously occurred. While at the accident scene, several witnesses testified that McPherson displayed unusual and biiarre behavior in her words and actions, and at some point removed all her clothing and began' walking naked along Belleview Blvd. Because of McPherson's unusual behavior she was placed in a Sungtar ambulance and treated by paramedics. Paramedic Bonnie Portolano testified McPherson told her she disrobed for attention so that someone would "help her". Portolano stated McPherson indicated she was having problems and wanted to talk to someone. Paramedics Bonnie Portolano and Mark Fabyanic transported McPherson to Morton Plant Hospital. McPherson was medically cleared by the attending physician at Morton Plant Emergency, Dr. Lovett and was to be evaluated by psychiatric nurse practitioner, RN Joseph Price prior to her release.

While McPherson was waiting to be evaluated several members of the Church of Scientology arrived at Morton Plant Hospital. Psychiatric RN Price believes McPherson's responses to questions were influenced by the presence of other Scientologists in the room who were within "ear shot"

of McPherson during her psychological evaluation.'Approximately ten members of the Ch.urch'of Scientology showed up at the hospital,* members of that group who hav6 been: identified are:

Dr. Lovett released McPherson against his medical advice, after Lisa signed the release form. Alain Kartuzinski indicated he drove McPherson to the Fort Harrison Hotel from Morton Plant Hospital to receive rest and relaxation and had no further contact with her until the day of her death.

On December 5, 1995, McPherson was transported by Scientologists Janis Johnson, Laura Arrunada and Paul Greenwood to Columbia HCA Hospital in New Port Richey where she was pronounced dead on arrival.

The Medical Examiner Joan Wood, MD indicates that'McPherson's immediate cause of death was a thrombo-embolism of the left pulmonary artery due to thrombosis of left popliteal vein due to bed rest and severe dehydration. It was also noted by Wood that McPherson suffered from severe dehydration, multiple old and recent hematomas and an abrasion to the nose.

INITIAL INTERVIEWS * Initial interviews by Clearwater Police Detectives on December 6, 1995, with individuals involved in the care of Lisa McPherson revealed the following:

Laura Arrunada: Arrunada was an assistant to Janis Johnson in the Medical Liaison Office of the Church of Scientology. Arrunada stated during previous contact with McPherson she (McPherson),was suffering from throat pain and had lost a considerable amount of weight. On the evening of 12/5/95 at approximately 1800 hours Arrunada entered McPherson's hotel room (reported to have been room 174) and McPherson was not talking but motioned to Arrunada she was still suffering from a sore throat. McPherson's skin was pale, and she had defecated in her undergarments and it appeared as if her condition had worsened. Arrunada contacted Medial Liaison Officer Janis Johnson. Arrunada stated the victim's pulse began to slow on the trip to Columbia Hospital and was fionresponsive upon arrival, at approximately 2130 hours, at New Port Richey Hospital. Subject Arrunada was deposed by the Pinellas County State Attorney's Office as a result of this investigation. For details of that later interview see page 32 of the witness statements in this document.

Alain Kartuzinski: Kartuzinski stated he is the supervisor of Lisa's counselor. Kartuzinski denies being Lisa's case supervisor. According to Kartuzinski Lisa was an acquaintance for about two years, but he didn't know her very well. Kartuzinski stated as far as he knew Lisa was a resident of 210 S. Ft. Harrison Ave. He further stated, at Morton Plant Hospital Lisa asked him for a ride back to the Ft. Harrison Hotel so she could rest. The last time he saw Lisa was on November 18, 1995, ' when he dropped her off at the Cabanas. Kartuzinski could not remember who Lisa's spiritual counselor was. He saw no injuries on her body at Morton Plant Hospital.

Janis J116hrisom. Johnson's initial statement indicated she received a call from Supervisor Alain Kartuzinski. Upon arrival at McPherson's room, McPherson was awake saying 'Hello" to Johnson and indicated her (McPherson's) throat was hurting. Johnson stated it appeared as if McPherson was septic. Johnson is a non-practicing Medical Doctor from the State of Arizona. Johnson called Dr. Minkoff who advised Johnson to bring the patient to New Port Richey.
