When requesting a copy of documents , doctors should have you sign this . Doctor was not able to provide me a copy of my signature that showed I would be charged this $25 ? and then Charts/records are to be kept for five years to prevent this issue , this doctor is purely in greed as the storage fee is either made up or looking for a way to make monday.
